You are ordered to appear for jury service. Report to the Some County Circuit Court building, Jury Assembly Room, 40 North Main Street, 5th floor, Some City, Some State, 12345 at the date and time printed above. You will serve for one day or one trial whichever is longer. Please bring this summons with you when you report for jury duty. If you must request excusal or postponement for any reason, please include a written request and supporting documentation (i.e. doctor note, copy or airline ticket, hotel reservation confirmation, letter from employer, etc.) along with your complete questionnaire and return it.

Please call 123-456-7890 and press 1 after 5:00 PM the day before you are scheduled to report. You may also visit www.somecityss.gov/clerksoffice/juryroom.asp to verify your attendance. You will be requested to appear based on your service number indicated above.

If illness or emergency prevents your attendance, notify the Jury Clerk between 8:00 A.M. and 8:30 A.M. at 123-455-1234 or e-mail: Juryroom@somecountyss.gov. Documentation will be required.

Everyone entering the Courthouse must go through security screening. Purses and packages will be X-rayed. You may be searched. Do not bring any weapons, dangerous objects or explosives into the building. Jurors must dress appropriately. Jeans, halter-tops, cut-offs, etc., are not allowed.

The State Supreme Court requires you to answer the enclosed questionnaires. Please mail the completed questionnaires to the court in the enclosed envelope within 10 days. If you wish to ask for postponement or excusal, for any reason, enclose a letter with the completed questionnaires and supporting documentation explaining the reason for your request and stating an alternate time for service. Mail your questionnaires even if you are requesting to be excused or postponed. You should receive a reply before your appearance date, but if you do not, you must report as ordered. Failure to return the juror questionnaires is considered contempt of Court, MCR 2.510 (B) (2).
